You will need to submit a completed Application for a Disability Placard or License Plate (REG 195) signed by your medical provider. You will be asked to upload this form during the online application process.
Please prepare one of the following acceptable documents to confirm your true full name and date of birth:
State-issued driver's license or ID card.
U.S. birth certificate
Valid, unexpired U.S. passport or U.S. passport card
Additional date of birth/legal identification documents may be used as valid proof of true full name and date of birth, if needed:

Be prepared to:
Attach the above documents to the appropriate section of this web form.
Pay the associated fee using your bank information (routing and account numbers) or credit card.
This application is for a new pass only. If you lose your pass or pass identification card, you may request a replacement online.
